NEW Idaho School Immunization Exemption Form

The Idaho Department of Health and Welfare has made additional revisions to the Idaho school immunization exemption form since the beginning of the 2017-2018 school year.

Please see the attached letter from Idaho Department of Health and Welfare for an explanation of the new form (attached).
